headspace provides early intervention mental health services to 12-25 year olds. The service is free or low cost and is designed to provide early access for young people and their families to receive the help they need for problems affecting their wellbeing. The service offers a holistic approach to supporting young people through four core areas: mental health, physical (including sexual) health, alcohol and other drug services, and work and study support. headspace delivers services through a Consortium model with a diverse range of specialized staff based at the centre providing services in the four core areas. headspace promotes early help seeking behaviour, contributes to mental health literacy as well as reduced stigma associated with youth mental health.

The headspace Pilbara Outreach Trial supports young people aged 12-25 with mental health, alcohol and drug services, physical and sexual health and vocational/education services. These elements come together within the consortium composition. Using an outreach and community engagement model, headspace Pilbara staff is co-located across various youth friendly locations.

The Health Navigator service promotes self-management skills to people living with chronic conditions through a virtual service, as required, utilising a modified Flinders My Health Plan.
The service operates in the Great Southern, Wheatbelt and South West. Client eligibility is for heart disease, heart failure, COPD, diabetes or those at risk of chronic conditions.

This is achieved through outreach counselling at the Street Doctor van in Wellington Street, along with case management and care planning for homeless people, through in-reach to residential treatment services and inner-city homeless services.

This Activity supports individuals who have been discharged from the care of the Emergency Department or Mental Health Ward to connect with primary care and other necessary community supports including mental health and alcohol and other drug services in the vulnerable weeks following discharge. Peer support workers work with people discharged from St John of God Midland Public Hospital to improve coordination of care to ensure people receive the right care in the right place at the right time. The activity improves health equity and access by identifying and reducing barriers to people with serious mental illness through short-term person centred peer support.

The Integrated Chronic Disease Care program provides clinical care coordination for vulnerable and disadvantaged persons who have a chronic condition such as diabetes, respiratory and/or cardiovascular disease. using a multi-disciplinary team-based approach involving GP referral, care navigation and access to health professionals, that is supported by telehealth services in remote locations.
