The Continuing Care program targets all Kimberley residents exiting residential or home based rehabilitation. It supports people recovering from alcohol and other drugs including methamphetamine. The program provides a holistic and culturally secure continuing care (step down) program for consumers. Continuing care teams provide sustained and assertive follow-up that includes the provision of monitoring and support to consumers who are transitioning from residential rehabilitation back to their normal environments. Individualised Care packages are developed and implemented as part of the consumers “Recovery Care Plan”.

Primary Care at Home (PCAH) aims to improve access to primary health care for vulnerable people. Nurse practitioners work with clients of selected community and social service organisations to undertake a health assessment, develop an individualised plan and link the person to a regular GP. Nurse practitioners go to the client’s home and work collaboratively with the client’s community support worker.
The role of PCAH is to provide primary health care to eligible community service organisation (CSO) clients in collaboration with the client’s community support worker. The primary health care is delivered by nurse practitioners, nurses, or allied health employed by (or contracted to) the Silver Chain Group.

Derby Abroiginal Health Service leads a strategy in collaboration with Numbala Nunga Residential Aged Care Facility and Derby Hospital to minimise after hours presentations to hospital by palliative patients receiving care in the community and Numbala Nunga Residential Aged Care Facility (RACF) residents.

The Primary Health Nurse Practitioner (PHNP) focuses on the management of complex chronic clients within community and residential settings. They work and have representation on the Chronic Conditions Collaborative Committees within the Great Southern and Eastern Wheatbelt, work collaboratively with the Chronic Condition client group in the strategy towards a common referral pathway through the Silver Chain HNAV program. In the absence of a GP they also support primary health access to these communities where they are supporting WACHS services in mental and Aboriginal health clinics.
