The activity increases the clinical capacity of Raphael Services’ existing multidisciplinary team, by resourcing mental health nurses. The mental health nurses work across the service locations including the Wembley hub, and satellite service locations in Joondalup, Merriwa and Midland in PHN Perth North; and Cockburn, in PHN Perth South. During the contract period options to establish a satellite location or outreach service in the Gosnells/Armadale area (PHN Perth South) will be explored.

The pharmacist delivers clinical pharmacy and education services tailored to the needs of the individual General Practice (using the Pharmaceutical Society of Australia (PSA) needs assessment protocol) through a coordinated, collaborative and integrated approach. The pharmacist also works closely with and provides a key point of liaison for community pharmacies to ensure continuity of care.
