The Mental Health Portal provides a stepped care approach for people with, or at risk of, mental health conditions that is comprised of core interacting components: a) Clinical care coordination b) Low intensity and psychological therapy phone-based and web-based services – Virtual Clinic c) Place-based community treatment services including structured low-intensity face-to-face services and psychological therapies, that in the absence of a negotiated pathway, are for individuals who are at low risk of suicide.

The Mental Health Primary Care Coordination (MHPCC) activity funds organisations to engage mental health care coordinators to help provide coordinated clinical care for people with complex mental illness and to minimise preventable hospitalisation.

The Kimberley Aboriginal Law and Culture Centre (KALACC) provide an on country, cultural program to build individual, family and community resilience and well-being.

The activity provides treatment and support services to people and/or their families who have alcohol and other drug (AOD) use issues who would otherwise experience significant barriers to engage in existing services provided by North East Metropolitan Community Alcohol and Drug Service to reduce the impact on individuals, families and communities.

This activity is a peer-based early intervention aimed at motivating vulnerable and disadvantaged people to change their behaviour associated with alcohol and drug use, and support them to access flexible and culturally appropriate primary, alcohol and other drug, and mental health services. Peer workers provide brief interventions, harm reduction and health education, mentoring and advocacy specifically for culturally and linguistically diverse people in the suburbs of Mirrabooka, Girrawheen, Koondoola, Balga and Westminster.
