Activity Work Plans

Our work plans outline the activities we will deliver to address inequity and provide for the local health and support needs of Western Australians.

WA Primary Health Alliance develops Activity Work Plans for WA’s three Primary Health Networks (PHNs): Country WA, Perth North and Perth South. It does this as part of the approval process for the use of the PHN funding provided by the Australian Government Department of Health and Aged Care.

These activity work plans are informed by the PHN’s Needs Assessment in which key issues and priorities are identified and aligned with the strategic direction of the organisation. We provide regular reports back to the Department on our progress in implementing these plans including any deviation from the plans.

In line with our contractual agreements with the Department, we publish all Activity Work Plans on our website after they have been approved. This provides visibility on our planned activities and management of PHN funding.

The Department of Health and Aged Care has developed a series of factsheets addressing frequently asked questions about the PHN program which you can view on the Australian Government Department of Health and Aged Care website.

Please see all current approved plans below.

If you have any questions about these plans, please contact us.
