Understanding and Using HealthPathways

In 2024, the HealthPathways WA team will run three demonstrations facilitated by a GP Clinical Editor. These sessions will demonstrate how to maximise integration of HealthPathways into your clinical practice.

HealthPathways WA is a secure, free-to-access clinical support tool available to general practitioners (and other health professionals) in Western Australia. Condition-specific pathways summarise best practice for assessing, managing and referring patients across WA.

This session has been approved for 1 CPD Educational Hour with the RACGP (activity #807950).

Pre-activity (15 minutes):

  • Complete the session specific activity – this will be emailed to you two weeks prior to your nominated session.

Demonstration (30 minutes), you will learn how to:

  • Easily access and use the platform to support your clinical practice.
  • Access local referral pathways, including clinical and service information.
  • Access condition specific tools, resources and Referral Access Criteria (RAC) (where available), as well as GPbook and visiting specialist rosters.

Questions (15 minutes):

  • Our GP Clinical Editor will be available to answer any questions from your pre-activity.

Learning Outcomes: 

  • Demonstrate the ability to use HealthPathways for assessing, managing and referring patients with a variety of complex presentations.
  • Identify two resources within HealthPathways to support evidence based best practice and population health.

Demonstrations are open to General Practitioners, general practice staff and other health professionals registered to practice in WA.

If you are unable to attend this session, our final demonstration will be held on the 14th November.
