SMHS GP Engage - Care of older adults

Fiona Stanley Fremantle Hospitals Group’s GP Engage program events provide valuable opportunities for local GPs, nurse practitioners and other primary care professionals to learn about hospital priorities, processes and programs affecting shared care.

This in-person networking and education event  will look at the patient journey for older adults – from the community, into tertiary care and then returning to the community.

Event highlights include:

  • An overview of geriatric services at Fiona Stanley and Fremantle hospitals
  • Insights into the patient journey, from admission to discharge
  • Best practice in the care of older adults
  • Q&A session with geriatricians.

Participation in this event can be recorded for your CPD via Quick Log (approximately 3 hours). Total CPD hours will be confirmed closer to the date.

CPD content (self-logged) may be recorded after each event, with an attendance certificate provided.
