My Health Record – Osborne Park Information Session

You are invited to attend a FREE information session to learn about the benefits of My Health Record.

Every Australian will have a digital My Health Record, unless they actively choose not to before 31 January 2019.

My Health Record is an online summary of your health information. You control what goes into your record, and who can access it. Share your health information with doctors, hospitals and other healthcare providers anywhere, anytime.

My Health Record can digitally store your important health information like allergies, current conditions and treatments, medication details, pathology reports or diagnostic imaging reports.

The session is hosted by WA Primary Health Alliance, don’t miss this opportunity to find out all you need to know about My Health Record.

During the opt out period – 16 July 2018 to 31 January 2019 – individuals who do not want a record will be able to opt out by visiting the My Health Record website or by calling 1800 723 471.

For more information or to create your own My Health Record online visit www.myhealthrecord.gov.au or call 1800 723 471.
